This is our medicine

The next meeting of the FSB blogger club took place. We talked about medicine. About Vyatka medicine and more. I decided to present the report of the meeting as my own opinion. I'll explain why at the end.

Firstly. Personnel problem. Of course, everyone knows about it and everyone is talking about it, including in the Kirov government. But the fact that everything is so neglected and the health department is virtually powerless in any, even the most insignificant, positive resolution of this problem is a discouraging reality.

The overwhelming majority of graduates of the medical academy do not want to go to the countryside to work, even if they are offered apartments and large apartments. I myself saw a rural first aid station where a woman works as a doctor, and only because her child was shown clean air and village food. And this is the determining reason. It turns out that you need to look for specialists who have sick children? Absurd.

And it is clear that if there are no doctors and teachers in a village or regional center, then life leaves such a settlement. Development stops. In 10-15 years, only old people will live here. And there are more and more such districts in our region.

The problem with personnel in cities is also acute. For example, reformers have reorganized existing medical institutions and are now forming so-called diagnostic centers on the basis of some of them. But specialists don’t go there - the salaries are meager. Good doctors go into paid medicine, and young and inexperienced doctors only disqualify their plans with their work.

Creation of an outsourced ambulance service and at the same time the formation of a paid ambulance service. Now imagine that if a businessman has invested in an ambulance and wants to earn money, and preferably more, then how will he process incoming calls. He will accept the “good” ones, but will refuse the “bad” ones under any pretext and advise the person to go to a paid ambulance.

What will we get as a result? It's scary to even think about.

The old has been destroyed, but it is impossible to build a new one.

Second. The system of government procurement of medical equipment, medicines and construction is in the hands of a few people. It often happens that hospitals order one specification, and some analogues are purchased, but either their quality is low or the functionality does not correspond to the application. But the price remained high. It is no secret that such a system is a good breeding ground for abuse and corruption. Apparently, multi-billion dollar amounts of financial flows are so attractive that officials have no strength to resist building various schemes that can direct a considerable part of budget funds to satisfy the ever-increasing personal needs of some of them.

Third. If you hear soothing words from management about some successes and achievements, don’t believe it. In behind-the-scenes conversations, officials themselves say that the planned modernization of healthcare in many places and moments is not thought out and is not supported by appropriate resources. This means that all changes and experiments, all modernization and reform will be and are being done “alive and without anesthesia” - bear with it if you can. And people endure.

Or, if change fails, then window dressing and statistical games begin to work. Such as visiting meetings of deputies in hospitals, where the main problems are the arrangement of personal parking lots at hospitals and the lack of doctors during the organization of sporting events. Health officials are not stupid people, they all know this, understand everything and absolutely consciously implement such a policy.

Fourth. In order to implement our plans, it is necessary to create a structure that will unquestioningly carry out even the most cannibalistic decisions.

Heads of medical institutions in case of any doubts, objections or failure to comply with management instructions receive penalties, reprimands or are dismissed without unnecessary ceremony, despite the existing staff shortage, which was mentioned above. A system of fear and unquestioning obedience is being implanted among chief doctors and leaders.

This is precisely why I present this material as my own point of view, and do not quote the statements of specific experts.

As a result, we have a system built as a functional economic model, in which a person and assistance to him are only one, and not the most important, element.

Such a system, as the doctors themselves admit, has actually destroyed the model of Soviet medicine, recognized as one of the best in the world, and due to the lack of money and experience, it is unable to convert into an effective and competitive system of medical care for the country’s population, as originally conceived.

As a result, against the backdrop of a demographic decline and a decline in the nation’s health, we will receive a medical care system that will only strengthen these negative trends, that is, simply contribute to our further degradation.
