“Guys, hello everyone! I am a doctor-an anesthetist-resuscitator, I work in a small hospital in the north of the region. The city of Severouralsk. At the moment, I am acting as the head of the intensive care unit ”-Alexei Parshukov, a 33-year-old resident of the Sverdlovsk region, began his desperate appeal in social networks in social networks.
“My patients are the most expensive,” the resuscitator continues. - Their treatment flies into a penny. Now many hospitals have multimillion -dollar debts. Medicine, equipment, consumables - everything is available at a minimum. And besides expensive drugs, my patients should eat. And some must be fed intravenously. This, in fact, is the problem - there is no parenteral (intravenous) nutrition in the hospital.
Without receiving protein and energy, there are less and less chances to survive every day. There is nothing to build tissues, cells to combat microbes.
In such a situation, no antibiotic will save. A weakened or killed microbe must also be digested and restored the fabric. On low protein, the liquid is not kept in the vessels, and infusion therapy turns into drowning.
On the morning line did not receive an answer to the question about parenteral nutrition and albumin. My question melted in the silence of the assembly hall. During the day, medicines never appeared. After work, I bought Albumin at the pharmacy, returned to work and appointed patients. There is no albumin for tomorrow. And parenteral nutrition is generally impossible to buy in a pharmacy ... There is no hopes for a miracle and the appearance of the above funds in the hospital.

A few months ago, I had to buy an albumin, and later wait for a refund for several months. This time I will not go to the management for reimbursing money. Let it be charity. But I ask you to respond to some charity fund, an organization to help in purchasing medical nutrition ...
... I have been disgusting to me lately from my helplessness. When I have nothing to help at work [patients]. But worse, when I go along the street after work and examine the election posters: “worries”, “can handle it” ... Only for some reason the governor, having received a carriage of reports of agony in Severouralian medicine, meets the miners and promises that everything will be corrected within three years. I am afraid that the hospital can bend in the coming months. No, formally medicine will remain, but it will be a feldsher-midwife point. ”
Severouralsk is a small city, about 25 thousand people officially live in it, and the number of residents is declining annually. Parts of local health care contribute to this. Despite the fact that today all the difficulties of medicine are habitually write off on Covid, questions have been rushed for a long time.
“Imagine the situation: there is only one traumatologist for the city,” Aleksey Parshukov gives an example in a conversation with a new correspondent. - For four months, he spent all his work alone: operations, and reception in the clinic, and duty almost without a break. It is clear that once this was supposed to end, a person cannot work constantly in this mode. He quit, for a month in Severouralsk there is no traumatologist. There are surgeons, of course, but they are still not traumatologists, they have their own specifics of work. It’s good that nothing serious happens, like an accident in a mine. We can not take out a helicopter of disaster medicine by a helicopter. There are situations when you need to help the victim here and now.
- You are not afraid that you will be fired after such an appeal to the public? - I ask Alexei.
“There are two options in this situation: silently observe what is happening or try to somehow change the situation,” our interlocutor replies. - I see how my peer is suffering in intensive care, and I can’t help him with anything. This is wrong. Perhaps I risk my reputation and career, but there is still a point to try to move the matter from the dead. I have an understanding that I am doing this not in vain. The key problem in medicine is a lie from top to bottom, everyone is trying to embellish something, give up, show that everything is fine. Yes, until some point it works until everything is collapsed at all. When one doctor works for the three, it seems to be not bad: both the staff is equipped, and the patients receive some help-and then he leaves, or gets sick, or even worse ... All came!
Alexei Parshukov has been working in medicine since student: six years old as a paramedic on an ambulance and four years in intensive care. Most of his earnings are duty. In the last month, he was able to earn more than fifty thousand rubles, does not complain about the lack of money. But to buy medicines, each ampoule of which costs more than two thousand rubles, from its salary is unlikely to be able to have long. And parenteral nutrition for patients cannot be bought at all, the doctor does not know analogues in Russian pharmaceuticals.

According to the head physician of the Central City Hospital of Severouralsk Andrei Naumkin, on the eve of social networks there was information from a resuscitator of Parshukov that there is a lack of protein drugs to provide parenteral nutrition for resuscitation department. The doctor expressed his private opinion, which does not coincide with the real situation in the hospital: the provision of medical care is carried out according to standards using various types of drugs, including analogues. The administration of the hospital monitors the availability of medicines in the departments of the hospital and purchases medicines as necessary.
According to the press secretary of the regional Ministry of Health Konstantin Shestakov, the Minister was appointed by the Minister. The Commission of the Ministry of Health goes to Severouralsk to understand the situation on the spot.
“If this [told by Parshukov] is true, then this is just a egregious situation,” the Sverdlovsk governor Yevgeny Kuyvashev wrote on his Instagram.
- I know that this drug is in the region, all hospitals should purchase it as necessary. I cannot imagine a reason justifying why this was not done in Severouralsk. I sent to the city of Deputy Minister of Health. He had already reached Severouralsk and began to understand the situation. I think that by tomorrow we will understand what is happening and why the doctor should buy medicines for patients for his own small salary. ”
It is worth noting that at the weekend the governor visited Severouralsk, where he was informed about the problem with personnel in a local hospital. According to Evgeny Kuyvashev, 123 employees, including 49 doctors, are not enough in the city healthcare institution.
According to the press service of the Severouralsk administration, last week at a meeting with Denis Demidov, Deputy Minister of Regional Health, a member of the Public Chamber of the Severouralsky City District Sergey Vasiliev voiced the problem with medical personnel. He also said that for several years in the North Urals CGB there was a shortage of drugs and consumables: bandages, syringes, adhesives, etc. Most of the free drugs approved by the Governor of the Sverdlovsk region are absent in the Severouralsk hospital. Patients with oncology have to go to chemotherapy to neighboring Krasnoturinsk.
